Digital interventions to support people with multiple health conditions


Abstract
Multimorbidity (suffering from multiple chronic health conditions) is a growing global challenge with substantial effects on individuals, carers and society.

Many chronic conditions can be better managed by patients themselves, however, people with multimorbidity struggle to self-manage their health conditions, which includes difficulties prioritising one condition over another, management of multiple medication regimes and adherence to lifestyle changes (exercise, diet).

The aim of this research is to understand the various needs, priorities and challenges of people with multimorbidity (and other key stakeholders) in understanding and managing their health conditions and propose possible digital interventions to support them.

This project will benefit from collaboration with the NWL Diabetes Team at Imperial and the Stroke Research Patients and Family Group based at King’s College London.
